The areas that are left white resist the dyes because that part of the fabric has been bound so no dye gets to it. The patterns come from the way that you bind, fold, twist, or clamp the fabric before dyeing it. The tiniest dot flower shapes come from tying them with thread many are less than 1 4" wide.
