Abstract In this systematic review and meta-analysis, we compared the efficacy and safety of tirzepatide with those of long-acting or ultra - long-acting insulin for type 2 diabetes

For instance, in the SURPASS-4 trial, where tirzepatide was compared with insulin glargine in patients with T2DM and a high cardiovascular risk, 15 mg of tirzepatide reduced the levels of TC, LDL-C, and TGs by 5.6%, 7.9%, and 22.5%, respectively, and increased the levels of HDL-C by 10.8%.5 In the SURPASS-5 trial, where tirzepatide was added to basal insulin treatment, tirzepatide also showed favorable effects on lipid profile parameters compared with placebo in patients with T2DM.6 Specifically, tirzepatide reduced the levels of TC, LDL-C, and TGs by 12.9%, 15.5%, and 24.9%, respectively, and increased the levels of HDL-C by 0.9%.6 In both trials, the effects of tirzepatide on lipid profile were dose-dependent, with greater improvements observed at higher doses and these improvements were sustained over the duration of the trials.5, 6 Table 1 summarizes the main findings of the SURPASS programs regarding the ability of tirzepatide to improve atherogenic dyslipidemia
